Chronic Disease and Disability Benefits Program

About This Program: 

The Chronic Disease and Disability Benefits program provides coverage for Yukon residents who have a chronic disease or disability and are under the age of 65. Your prescriber applies to the program for you. In the application, they will include your chronic condition and any needed medications or equipment. 

What can you get help with costs for? 

You may be able to get help with paying for: 

  • prescription drugs; 
  • medical surgical supplies; 
  • medical equipment; 
  • food supplements; and 
  • prostheses. 

You can get help with these costs if: 

  • these are things that help you manage your chronic disease or disability; and 
  • they’re recommended by your health practitioner. 

You won’t receive benefits if: 

  • you’re eligible for full benefits through private or group insurance, or you are a resident of a federal or territorial facility or institution. 
  • medical equipment can be borrowed from a hospital or the Canadian Red Cross.

How to Apply/Application:  

Your doctor must apply to the program for you. If you live in a community without a resident doctor, a community health nurse can apply for you. 

Normally, the application is made before a purchase, but sometimes this isn’t possible. To be reimbursed, you’ll need to submit a claim within a year of the purchase. With your claim, you’ll need to include: 

  • full documentation; and 
  • recommendation of a qualified, regulated health professional 

If your application is approved, you’ll still need to pay a $250 deductible, or the first $250 of eligible costs each year, to a maximum of $500 per family.

Chronic Conditions Support Program

About This Program: 

The Chronic Conditions Support Program (CCSP) is a patient-centred, collaborative care team. It offers a variety of programs and services for Yukoners living with chronic health conditions. Choose which programs and services will help you live well and make plans for the future. 

All programs and services are free and available by self-referral to Yukoners age 18 and older. We recommend you speak to your doctor if you have any concerns about signing up. 

Programs and Services: 

  • Nurse educator support 
  • Exercise services 
  • Pulmonary rehabilitation 
  • Home health monitoring 
  • Group education and support programs

Children’s Disability Services

About This Program: 

If your child has a disability, Children’s Disability Services can help you care for them. It’s important to get help early as this will increase your child’s lifelong learning potential. We promote inclusion of children with disabilities in community life. Our social workers help families access the services they need. These services include: 

  • an inclusion worker; 
  • respite care for families; 
  • a behaviour consultant and applied behaviour analysis therapy; 
  • occupational therapy and physical therapy; and 
  • speech-language pathology (therapy). 

You’re eligible if: 

  • you’re a Yukon resident; 
  • your child is under 19 years of age; and 
  • your child has a disability that makes it difficult for them to do typical, day-to-day activities. 

How to Apply/Application:  

1. You or someone working with your child, such as a doctor or teacher, can make a referral with your permission. 

2. To help determine eligibility, you (or the person referring) must provide your child’s: 

  • full name; 
  • date of birth; 
  • type of disability (if known); 
  • caregiver names; and 
  • relevant reports.

3. A social worker arranges a meeting to discuss your family’s needs. Visits can take place at your home, the Children’s Disability Services office, or elsewhere. 

4. This initial meeting lasts about 1 hour. A follow-up meeting may be needed. During the meeting, the social worker will: 

  • gather information about your child’s needs; 
  • review consent and confidentiality forms; 
  • discuss alternate supports and services; and 
  • share information about the supports and services we offer.

5. We’ll also need a report or assessment. This will identify how your child’s disability significantly limits their ability to function in daily life. 

6. The social worker will contact you to follow up. 

7. If your child and family are eligible, it may take 4 to 6 weeks to set up the funding.
